Cleveland, Ohio clinic performs US’s first face transplant

A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. The patient, however, must take lifetime immunosuppressive drugs, also called antirejection drugs, which do not guarantee success. The transplant team said that in case of failure, it would replace the part with a skin graft taken from her own body.

Dr. Bohdan Pomahac, a Brigham and Women’s Hospital surgeon praised the recent medical development. “There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.

On Monday, 46-year-old Dolores O’Riordan, lead singer of Irish rock band The Cranberries, was found dead, her publicist Lindsey Holmes confirmed; reports suggested she was found in her room at the London Hilton on Park Lane hotel. O’Riordan’s agent said she was in London, England, for a recording session. According to the police, her death was considered “unexplained” and no cause of death disclosed, though it was to be investigated.

Later one of the richest women of Ireland, O’Riordan joined the rock band, then known as “the Cranberry Saw Us”, as a teenager around 1989, after Niall Quinn had left the band. Also featuring drummer Fergal Lawler, bassist Mike Hogan and his brother Noel Hogan, the lead guitarist, the band released their first album Everybody Else Is Doing It, So Why Can’t We? in 1993. The debut album featuring single Linger was an international success. The band went on to release six more albums featuring O’Riordan’s voice, including Something Else, which was released last year.

A year later, in 1994, the band released their second studio album, No Need to Argue, which featured the song Zombie, a song about bombings related to Northern Ireland. Zombie topped the singles charts in various countries. In the same year, O’Riordan married Don Burton, sometime manager of the Duran Duran band. Their third album, To the Faithful Departed, was released in 1996. The album did not receive the acclamation of the first two.

In 2003, the band split, and it was reunited in 2009. During those six years, O’Riordan released two solo albums — Are You Listening? in 2007 and No Baggage in 2009. 2012’s Roses was the first album The Cranberries released after reunification.

Last year, O’Riordan revealed through London’s Metro newspaper she had been diagnosed with bipolar disorder. She also spoke of suffering from depression to The Irish News. In 2014, the singer was involved in assaulting a flight attendant and three police officials on a flight from New York, US to Ireland. She was fined €6000 (US$6600) after she pleaded guilty; medical reports said she suffered from mental illness during that incident. Last year, the band’s tour to North America and Europe for Something Else was cancelled partway through as O’Riordan suffered from a back problem.

After announcement of O’Riordan’s death, Irish prime minister Leo Varadkar tweeted, “For anyone who grew up in Ireland in the 1990s, Dolores O’Riordan was the voice of a generation. As the female lead singer of a hugely successful rock band, she blazed a trail and might just have been Limerick’s greatest ever rock star. RIP.”

The official twitter handle of The Cranberries posted a tweet saying, “We are devastated on the passing of our friend Dolores. She was an extraordinary talent and we feel very privileged to have been part of her life from 1989 when we started the Cranberries. The world has lost a true artist today.”

Born on September 6, 1971 in Ballybricken, a town in southeastern Ireland, O’Riordan had six older siblings. She wrote songs since the age of twelve. James Walton, a priest from her home area, said, “The plan is for her to be buried here at home. When that will be will depend on when her body is released.”

Dolores O’Riordan is survived by three children and ex-husband Don Burton.

20 confirmed dead after Kintampo freak tree accident

A freak accident caused twenty deaths and some injuries, according to officials, at the well-known Kintampo waterfall, in Ghana, on Sunday afternoon local time. Those affected were reportedly school students from Wenchi Senior High School on a supervised trip, and several tourists, who were swimming at the time of a rainstorm and strong winds, bringing down a tree and crushing victims.

Eighteen students, according to Ghana National Fire Service spokesman Prince Billy Anaglate, were killed at the falls, while another two were rushed to hospital and died in care. Varying reports indicated possibly up to 18 people remaining in a critical condition who were trapped under the tree, who were then rescued and rushed to hospital, including a Wenchi Senior High School administrator.

Anaglate said the wind uprooted trees, which fell on swimmers.

Students’ bodies have been sent to the Kintampo Municipal Hospital’s morgue.

Russia stages military exercises as Ukrainian forces advance

On Monday, as Ukrainian forces continue to advance into rebel held territory, Russia announced it is to hold military exercises near its border with Ukraine.

Russia said the exercises would involve over 100 aircraft. Western leaders have accused Russia of a military build-up adjacent to Ukraine. Russia held military exercises in the area in March; last week NATO General Philip Breedlove said Russia still has 12,000 troops adjacent to Ukraine, although Russia has claimed its forces have withdrawn from the area. Russia has also been accused of arming rebels in the continuing fighting in Eastern Ukraine.

Russia also said the exercises were to include an array of fighters, bombers and helicopters, and told Interfax the purpose of the exercises is to improve air force cohesion. This week’s exercises are to be followed by others with the same goal.

Meanwhile, fighting continued in Eastern Ukraine as government forces advance on the cities of Donetsk and Luhansk, the principal cities left in rebel hands. These cities have both been reported to be surrounded, with civilians stockpiling resources in preparation for a siege amid worsening conditions. The Telegraph reported groups of rebels arming themselves with a mixture of new and old weapons, while telling accounts of setbacks inflicted upon the Ukrainian army.

The fighting, ongoing since April, is reported to have increased in intensity since Malaysian Airlines Flight 17 was shot down last month. The cause of the crash is still being investigated with experts from the Netherlands and Australia continuing to search the crash site.

Pakistan’s ban on YouTube lifted

Pakistan has lifted a ban on the video-sharing website YouTube, which was said to be brought in after a video offensive to Islam was uploaded to the site. According to the Pakistan Telecommunication Authority (PTA), the offending material was in relation to a trailer for a film by Geert Wilders, a Dutch politician.

The PTA ordered the country’s 70 internet service providers to ban YouTube. However, their actions mistakenly blocked the site on other international ISPs, preventing many countries’ access to the popular site for around two hours.

Wilders is expected to release a movie about violence in Islamic culture. The film called Fitna would be set ‘inside’ the Koran, with a book frame surrounding the images and a mix of Koran verses with footage of executions and other violence in Islamic countries such as Iraq, Iran and Saudi Arabia.

In the United States, the Federal Bureau of Investigation (FBI) and the Department of Homeland Security (DHS) fear that the release of the movie might trigger a worldwide surge of violence. It is expected that the film will show the destruction of a Koran, which is “tantamount to heresy” in Islam, according to the FBI/DHS report. The Dutch Prime Minister has said that the movie has caused a serious crisis situation, with several Dutch embassies and companies abroad worrying about safety. Wilders has previously declared that the Koran should be banned, and he has compared it to Hitler‘s Mein Kampf.

The PTA has also blocked websites depicting the controversial cartoons of Muhammad published by a Danish newspaper in 2006 that also sparked violence in the Muslim world. Some of the cartoons have recently been reprinted.

This is not the first time a country has moved to block YouTube. Turkey and Thailand both chose to temporarily ban the sites, as did Morocco which blocked the site after a video criticising the country’s treatment of Western Saharans was uploaded.

Some of you may have had this experience: You are standing back stage at your associations annual conference ready to walk out and deliver the opening speech to the general assembly. On cue, you walk out to face 1,200 people waiting to be entertained, informed and inspired. But not to worry, youve had plenty of training in the art of giving good speeches; so you smile at the audience and begin

But more likely, you have had this experience: Youre walking down the hall on your way to a meeting and your boss stops you long enough to tell you that the Board of Directors is meeting next week to consider the new building proposal. Since you are one of the leads in this project, youll need to prepare and give a presentation to the Board.

Gone are the days when the need for good presentation skills applies only to the person standing in front of 1,200 people; rather, good presentation skills are required of the 1,200 people in the audience.

Consider the following:

Team Meetings: Your team can meet as a group only once week and it is your job to make sure the most relevant information is delivered in the most effective manner in the time you have to deliver it. Sure, you will meet with individuals and groups during the week, but this is your only chance to meet with the team together, and what you say and how you say it has to be good.

But why muddle when you can be good; and why be just good when you could be great? And is it really important to be a great presenter or speaker? What are the benefits of being great?

First, the ability to present or speak well creates an effective work group. People understand the information and direction and work gets done. Good presentation is directly related to accomplishment.

Second, good presentation saves time. When people understand information and direction, time is saved and thus created for additional accomplishment. You get more done in the time you have to do it.

The need for good presentation is everywhere; learn to present well, use it and benefit from it.
